This document is a legally binding agreement under South African law, which outlines the terms and conditions of your participation in an ice bath session and other recovery services provided by Metawell. I, the undersigned, hereby consent to participate in the following recovery services provided by Metawell. I understand that these activities involve physical exertion and/or physical touch, and that there are associated risks as described below.

I consent to receive recovery services (ice bath, massage, assisted stretching, etc.) from Metawell Recovery.
I understand these services are not medical treatments and do not replace professional healthcare.
I confirm I have no known medical conditions that would make these services unsafe for me.
I release Metawell, its staff, and affiliates from liability for any injury or adverse effects that may occur during or after my session.
